Trafford CIL’s Transitions project is a free service funded by BBC Children in Need. We work with 14—18 year olds who live in Trafford and consider themselves to have a long term health condition,  impairment, disability or are young carers.

What we offer

  • Advice and guidance to young people and their families when making choices about education, employment or independent living.
  • One-to-one emotional support
  • One-to-one personal goal setting sessions.
  • Opportunities to participate in fun activities and skills based workshops.
  • Signposting to other services.
  • Opportunities to participate in feedback forums to have your say about important issues.

Transitions activities

We run monthly fun and skills based workshops, along with activity sessions  during half terms. All our sessions are free of charge and provide an opportunity for young people to come together to socialise and make new friends.

How do I join Transitions?

Transitions is open to Trafford residents, aged 14 -18, who can self refer or be referred by a family member or  a professional.
